Qualified Referral. A Qualified Referral is defined as a successful assessment at Canberra Eye Laser by a person (a "Referred Patient") who was also found to be suitable for laser eye surgery within the last 12 months (since July 1, 2018). You are limited to one Qualified Referral for each Referred Patient; in other words, additional/repeat referrals made by a Referred Patient are not counted as additional Qualified Referrals.
Referred Patient. The Referred Patient and the Referrer cannot be the same person (for example, by using a different email address). The Referred Patient has to be a new patient who has never had an assessment at Canberra Eye Laser before. We automatically disqualify suspicious referrals.
Referral Reward. For you to earn referral reward as a Referrer, the Referred Patient must complete an assessment and be found to be suitable by September 30 2019. The Referred Patient must also advise the reception or staff that they were referred by you.
Rewards. Rewards are payable in gift vouchers. The maximum Qualified Referrals earned per the calendar year may not be more than AUD$40.
Eligibility. Eligibility is limited to individuals only. Canberra Eye Laser’s Refer A Friend Program cannot be used by businesses.
Right to Refuse Referral Reward. Canberra Eye Laser reserves the right to refuse payment to any Referrer and/or Referred Patients if the Referrer and/or Referred Patient attempts to use the Canberra Eye Laser Refer A Friend Program in a questionable manner or breaches any of these Terms & Conditions or is in violation of any law, statute or governmental regulation.
Right to Cancel Program or Change Terms. Canberra Eye Laser reserves the right to cancel our Refer A Friend Program or to change these Terms & Conditions at any time in its sole discretion. Any unclaimed referral rewards will be forfeited at that time.
CONSULTATION TERMS & CONDITIONS
All consultations with clinical staff are free, as are any follow-up consultations with clinical staff or surgeons.
Obligation-free consultations are with clinical staff to determine viability for laser eye surgery.
If viable for laser eye surgery, surgery costs will be discussed within the consultation. Payment for surgery will not be made until day of surgery.
Patient has no obligation to book surgery with Canberra Eye Laser.
Laser eye surgery patients must be deemed medically fit and suitable for surgery, following a consultation with our clinical staff, and in some cases, our surgeons.
